Certified Lean Office Sustainment Program

Part of the Lean Healthcare Transformation Initiative

Indiana organizations/hospitals with lean offices certified by Purdue Healthcare Advisors may fill out this form to request additional INSORH-subsidized lean training to certify and train two Lean Practitioners (LPs) and two Lean Daily Improvement (LDI) Facilitators. 

  • LDI Facilitator training is not a pre-requisite for LP training.
  • Individuals who cannot make it to their assigned session must attend another.

Lean Practitioner (LP) Training Sessions

Training sessions delivered in person. Five options provided:

  • October 1-4
  • October 15-18
  • November 19-22
  • December 9-12
  • January 7-10

Candidates for Lean Practitioner (LP) Training
Lean Daily Improvement (LDI) Facilitator Training Sessions

Training and coaching sessions delivered remotely through project certification. Four modules over two days with break midway for homework completion. Two options provided:

  • November 11 & 18
  • January 20 & 27

Coaching Session

One-Day Coaching Session Session delivered in person with a schedule based on the urgency of your needs.

Thank you for taking the time to apply for the LHTI's Certified Offices Sustainment Program. PHA will send verification to all applicants and "cc" the main contact at each site by September 25th. Upon submission of this form, your main contact will receive an email confirming your application and listing your responses.
